We are located in the Arrowhead Hospital Medical complex in Glendale, Arizona

You may park in main visitor and patient parking lot directly off of 67th Avenue, on the northwest corner of the hospital. The medical complex in to the right of the main hospital entrance. Take the elevator to the 3rd floor and exit to your left when stepping off the elevator. Our office is suite 320 and will be directly in front of you after you turn left off the elevator.
